Using the Patient Portal

Logging in

To log in to the RSDM Patient Portal, you will use two-factor authentication. After you enter your email address and password, you will be asked to verify your identity with a one-time code sent to your email or phone.

We recommend using your mobile phone number for this step when possible. Email delivery may take several minutes, depending on your provider.

The first time you log in to the Patient Portal, you will be asked to review the legal disclaimers. Please scroll to the bottom, check "I have read and accept," and then select "Next." You will then be directed to the Patient Portal home screen.

Checking in for Your Appointment

After you log in to the Patient Portal, you will be directed to the home screen. From there, you can view upcoming and past appointments, access billing services, and download a copy of your health records.

To check in for an upcoming appointment, go to the "Ready for check-in" section on the home screen and select "Check in." Select the appointment you would like to check in for, and then select "Check in" again.

You will be asked to complete any required forms before your appointment. All patients must review and update these forms each year. New patients must complete the forms before their first visit.

Please complete the medical/dental history, insurance information, and patient registration forms.

When all required forms are complete, select "Submit." A confirmation message will appear after your forms have been received.

Reviewing Your Submitted Forms

To review information you previously submitted through a portal form, such as your medical history, select "Forms" from the main menu, and then click "Completed Forms."

A window will open where you can view or download your completed form.

Managing Your Bill

The "Billing" tab shows any remaining balances on your account. From the Billing page, you can review your outstanding balances and make a payment by credit card.

To make a payment, select "Pay" in the upper-right corner of the billing page. Enter the amount you would like to pay, and then enter your credit card information.

Please note that payments made through the Patient Portal are applied to your oldest outstanding balance first. The portal does not allow payments to be applied to a specific treatment.

Logging Out of the Portal

To log out of the Patient Portal, select your name above the navigation menu, and then click "Logout."
